While manual spamming can't be prevented without locking sensitive
pages so that they are admin-only, all automated spam has certain
footprints which can be detected and blocked by the Bad Behavior
extension:

I think I already implemented most of the basics way back when, but a
quick checkup wouldn't hurt.

Captcha might be a good idea for new users.  I've heard that one
captcha extension forces its use for new users until they make a
certain number of edits.  But I've done no research in that direction.

Also, above all else.. please keep the MediaWiki software updated.  If
PHP5 isn't available, then at least update to the latest 1.5.x just to
be safe.

Updating is a matter of backing everything up (to be safe), unpacking
the new version overtop and running 'php maintenance/update.php' I
believe.
