Could it be a flaky DSL modem? I've had similar (not identical) results
that went away with another modem. See if you have a neighbour or friend
with a DSL modem and borrow it for testing.

Other things to consider is interference from other phones in the house.
Pathcom must have taken you through this by disconnecting all the phones
(and answering machines and Asterisk boxes) in the house.
