Looking for a "your post sucks" response template

Hey, guys.

I just knooowww someone on this list has one of these.  I'm not certain what
their called but they usually (or used to) get posted in response to trolls
and the such and looked like:


    /************ SNIP  ******************/
   
    This form in response to your posting of XXX has been sent to inform
    you that you have:

        [ ] trolled
        [ ] spammed
        [ ] insulted
        [ ] asked for inappropriate help on your homework
        [ ] ....

    the entire group/list with your (select all that apply):

        [ ] rascist             [ ] slanderous
        [ ] homophobic          [ ] hateful
        [ ] incorrect           [ ] biased
        [ ] ...

    comments.  We put it to a vote and think that you should consider:

        [ ] entering the Darwin Awards
        [ ] posting a retraction
        [ ] going away
        [ ] ...

    ...
    /***************************************/

Anyone know what these are generally called and/or where to find a good
example?

PS - No, this is not a homework assignment :)
